New Student Online Registraion
The New Student Registration Form is for students that are not currently enrolled in the Park City School District (new to the district or not enrolled at the end of last school year)
Students that are new to the Park City School District will use the New Student Registration Form. You must fill out all required information and submit at the end of the registration process or your registration will not be complete.
Online Registration ENGLISH
Students will NOT start school until the Administrative School Secretary has received ALL required documents. It is the parent's responsibility to provide us with copies of the following documents to complete registration:
- Birth Certificate
- Immunization Record
- Proof of Residency
- Guardianship Papers

Registración En-Línea para Estudiante Nuevo de la escuela McPolin
La forma de registración de estudiante nuevo es para los estudiantes que no están actualmente matriculados en el distrito escolar de Park City (nuevos en el distrito o no han sido matriculados al fin del año escolar anterior.)
Los estudiantes que son nuevos en el distrito escolar de Park City usarán la forma de registración de estudiante nuevo. Usted necesita llenar toda la información requerida y someter al final de su proceso de su registración o su registración estará incompleta.
Registración En-Línea- ESPANOL
Los estudiantes NO empezarán la escuela hasta que la secretaria administrativa haya recibido TODOS los documentos necesarios. Es la responsabilidad de los padres presentar las siguientes copias de los documentos para completar la registración:
- Acta de nacimiento
- Cartilla de vacunas
- Comprobante de residencia
- Documentos de tutela
